Progressive Goal Attainment Program (PGAP)

PGAP is a 10-week program developed by a psychologist to be delivered by trained healthcare professionals to target behaviour change in relation to the psychosocial barriers that sustain disability and prevent return to work.

The clients establish realistic goals and a weekly activity schedule that brings “meaning and value” back into their lives with a focus on return-to-work planning in the later stages. This program is ideal for clients having difficulty motivating themselves to get moving. It is more effective in the earlier stages of recovery before behaviours associated with chronicity are fully entrenched. Assessments are completed at intake, midterm and discharge.

PGAP Goals:

  • Move away from symptom management toward increased active participation
  • Re-integrate into life roles while increasing independence and control
  • Overcome psychological obstacles through gradual and progressive change
  • Return to work

When to Consider PGAP:

  • Clients experiencing a high level of disability (pain, fatigue, depression) associated with a chronic health or mental health condition
  • All other serious medical conditions have been ruled out and other treatment options have not been successful
  • The individual considers rehabilitation goals as a priority for them
